If you are new to our practice, we encourage you to download and print out the new client forms.
Filling these out in advance of your first visit will minimize the time you spend in the waiting room filling them out when you get here.

- Parental Consent Form (Parental consent for treatment)

If you are wanting help, our counselor center provides individual, group, couples, and family counseling for anger management.
There are two ways to setup an appointment with one of our counselors:
